The 2010–11 First Macedonian Football League was the nineteenth season since its establishment. It began on 31 July 2010 and ended in May 2011. Renova were the defending champions having won their first Macedonian championship last season.

The competition was won by Shkëndija Tetovo, who earned their first overall Macedonian title. FK Skopje and Pelister Bitola were relegated to the Second League.

Teams

Three clubs from last season's competition were expelled from the league before the season had ended. Makedonija and Sloga Jugomagnat were expelled from the league when the clubs failed to appear in two consecutive matches in the competition. Later in the season, Pobeda were expelled from the league for their involvement in match fixing during the 2004–05 UEFA Champions League against Pyunik of Armenia. These three clubs were replaced by Macedonian Vtora Liga champions Škendija 79, runners-up Skopje and third place finisher Napredok.

In addition, one more spot in this season's competition was available via a playoff between the Prva Liga's ninth place finisher, Milano, and the Vtora Liga's fourth place finisher, Bregalnica Štip. Bregalnica Štip defeated Milano, 2–1 after extra time, and were promoted to the Prva Liga.

Results

The schedule consisted of three rounds. During the first two rounds, each team played each other once home and away for a total of 22 matches. The pairings of the third round were then set according to the standings after the first two rounds, giving every team a third game against each opponent for a total of 33 games per team.
